Beginning with your most recent place of employment (to include current employment), list the last five years of employment history with complete addresses, phone numbers and contact name(s).

Applications will not be considered without three (3) complete references, which must meet the following criteria:

• Name and phone number must be provided
• Must have at least five (5) year relationship with reference
• Cannot be related to you

I certify the answers I have provided in this application are true and complete to the best of my knowledge. I authorize investigation of all statements contained in this application for employment as determined necessary by Helen R. Tucker Adult Development Center in order to arrive at a decision regarding my employment. This application for employment, if complete, will be considered active for a period of six (6) months. Should I wish to be considered for employment  beyond this time period, I understand a new application must be completed and submitted to Helen R. Tucker Adult Developmental Center.

I hereby understand and acknowledge that, unless otherwise defined by applicable law, any employment relationship with Helen R. Tucker Adult Developmental Center is of an "at will" nature. This means I, as the employee, may resign at anytime without notice or cause; and Helen R. Tucker Adult Developmental Center, as the employer, may terminate my employment at anytime without notice or cause. It is further understood this "at will" employment relationship may not be changed by any written document or by conduct unless such change is specifically acknowledged in writing by myself and an authorized representative of Helen R. Tucker Adult Developmental Center.

I understand Helen R. Tucker Adult Developmental Center will conduct a criminal background check if I am considered for employment. I also understand if I receive an interview, it does not necessarily indicate I will be hired for the position I am applying for, or for any other position at Helen R. Tucker Adult Developmental Center. I also understand I am required to abide by all rules and regulations set forth by Helen R. Tucker Adult Developmental Center if I am hired.

I certify I have at least three (3) years driving experience, and I have lived in the state of Tennessee for at least the last one (1) calendar year.

I also understand if my application has not been completed in entirety, or honestly, I will not be considered for employment.
